This module focuses on the ongoing nature of case management, supporting people we work with beyond initial engagement by maintaining meaningful contact, tracking progress, and adapting plans as circumstances evolve. Participants will learn how to balance the dual responsibilities of compliance and person-centered care, ensuring that support remains responsive, relevant, and forward-looking.

Key themes include strategies for maintaining engagement over time, recognizing when and how to adjust service plans, and preparing for transitions, such as stepping down services, transferring care, or supporting greater independence. The module also explores how to navigate competing priorities, manage caseloads effectively, and ensure continuity of care.
