Holistic Assessment and Goal Setting Through Effective Home Visits

This practical, skill-building module equips participants with the tools and techniques needed to conduct comprehensive, person-centered assessments in the context of home visits. Recognizing the home as a vital space for understanding the lived experience of people we work with, this module emphasizes the importance of building rapport, observing with sensitivity, and engaging in collaborative planning.

Participants will learn how to identify needs, risks, and strengths through a holistic lens, considering physical, emotional, social, and environmental factors. The module also introduces SMART goal setting as a structured, empowering approach to support tenants/clients in defining and working toward meaningful change.

By the end of this module, participants will be able to approach home visits with confidence and care, using them as a powerful tool for engagement, assessment, and goal-oriented support.
