This module focuses on the essential role of the case manager as a connector, linking people we work with to community-based services and supports that align with their unique needs, identities, and goals. Participants will explore how to develop individualized service plans that are comprehensive as well as culturally responsive.

By the end of this module, participants will be equipped to serve as effective liaisons, ensuring that those they work with are meaningfully connected to the right supports at the right time.
